4. Arrest and/or Conviction of a Crime While a Volunteer

   Volunteers shall notify their immediate supervisor of any arrest, indictment or convictions with five (5) calendar days of such arrest, indictment or conviction. In most instances, the Human Resources Director will conduct an investigation and take appropriate action.  

   By signing below, I state I have received a copy of the above policy and I understand that it is my responsibility to comply with it when and if there is an occurrence. I also understand that not complying can lead to disciplinary action from management up to and including permanent disqualification from the City of The Colony's volunteer programs. 

6. Electronic Signature Agreement

   By checking the "I agree" box below, you agree and acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, and 3) you may still be required to provide a traditional signature at a later date.
